Sensibilidad y Especificidad de las Escalas Clínicas HEART, TIMI, GRACE, EDACS en la Evaluación del Infarto Agudo de Miocardio: Revisión Sistemática

Resumen

Introducción: El dolor torácico agudo supone un reto en urgencias: la mayoría de pacientes no presenta IAM, pero omitirlo tiene un alto costo clínico.

Objetivo: Sintetizar la exactitud diagnóstica (sensibilidad/especificidad, LR±, DOR, AUC/SROC) de HEART, TIMI, GRACE y EDACS en adultos con dolor torácico atendidos en urgencias, para identificar la estrategia con mejor balance seguridad–eficiencia para el descarte temprano de IAM.

Método: Revisión sistemática (PRISMA 2020/PRISMA-DTA) de estudios de 2019 a 2025 en PubMed, Scopus y Web of Science. Se incluyeron cohortes y estudios de DTA que reportaran pares Se/Sp o datos 2×2 para ≥1 escala en el episodio índice; evaluación de sesgo con QUADAS-2 y síntesis planificada con modelos bivariados/HSROC. Flujo PRISMA: identificados 910 registros; duplicados 490; cribados 420; excluidos por título/resumen 250; evaluados a texto completo 170; excluidos a texto completo 137; no recuperados 5; incluidos cualitativa 28; sin estimadores comparables 8; incluidos en metaanálisis 20.

Resultados: En conjuntos con hs-cTn y protocolos 0/1–0/2 h, HEART/HEART Pathway (≤3) mostró sistemáticamente altas sensibilidades y VPN muy elevados, adecuados para rule-out con especificidad moderada; EDACS-ADP mantuvo sensibilidades altas con ligera ganancia de especificidad y mayor proporción de altas tempranas cuando se cumple el algoritmo; GRACE aportó valor pronóstico (mortalidad/complicaciones) pero no superó a HEART/EDACS en el descarte inicial; TIMI alcanzó sensibilidades altas con baja especificidad a umbrales “bajo riesgo”, por lo que no es recomendable como única regla de alta en triaje indiferenciado.

Conclusiones: La estrategia preferida para el descarte temprano de IAM en urgencias es HEART Pathway (≤3) + hs-cTn con protocolo 0/1 hora; EDACS-ADP es una alternativa eficiente cuando se priorizan altas tempranas con laboratorio rápido y alta adherencia; GRACE debe usarse para la estratificación pronóstica posterior; TIMI no debe emplearse en solitario para decisiones de alta.
